Please note: Any updates need to be conveyed to BPP in writing via email at BPP@bmcjax.com as soon as possible. Updates include changes to the TIN or provider, practice location changes, and contact information changes.

We appreciate your interest in joining Baptist Physician Partners. Please enter your name and email address below. You will receive an email with application instructions and a link to our online application form.

Applicants are reviewed and considered for membership in Baptist Physician Partners on an individual basis. The criteria for membership includes, but is not limited to, the provider’s commitment to improve the health of patients served by the Network, provider’s qualifications, provider’s practice history in the community, and the Network geographic and specialty needs at the time of enrollment.

Your inclusion in BPP will become effective only after approval by the BPP Board of Managers. If approval is effective after April 1st of the current year, you will not be eligible for shared savings until the next performance year.

Please note: Locums and fellows are not accepted into Baptist Physician Partners at this time.
